On this web page we provide you with some of the most frequently requested forms that we are asked for in the Gateway. With each of these forms there is a brief note on:
- what the forms are for and
- how to fill them out.
|Form||Note||Return form to:|
|Change of Details Form||Used to make sure that your student record has the correct information, such as; your name and addresses are kept up-to-date. This form can be filled out and returned to the Gateway Enquiries Desk. If you are wishing to change your name you must bring original proof of this change, such as: Birth or Marriage Certificate, Deed Poll or other official documents.||The Gateway Enquiries Desk (City or St Peter's Campus)|
|Internal Transfer form||Internal Transfer Forms are for students who feel that their current course is not suited to them, and who want to move to another course at the University. When filling out this form it is stressed that you must talk to the Programme Leaders of your Current and New courses.||The Faculty Office of your new course|
|Leave of Absence form||
Every now and then students need to take time out of their course for many different reasons and using the is how you do this. As with the Transfer Form, you need to have your Programme Leader sign, saying that they agree with your decision.
Your Faculty Office
|Withdrawal Form||The Withdrawal form is for those who find that they can no longer stay on their course for whatever reason, and who do not wish to return to their studies. This form requires the signature of your Programme Leader. There is also a section which should be completed, if you are living in University accommodation - just to let them know.||Your Faculty Office|
